1. MedRemind: fully offline for the things that matter

MedRemind is the only app we tested whose reminders, dose logs, drug reference, and drug interaction checker all work with the device in airplane mode. The app ships with the openFDA label database and Spain's CIMA database built into the app bundle. When you look up a medication, the detail comes from the device, not a server. The interaction check runs on the device too.

2. MedTimer: offline by design

MedTimer collects no data and makes no network calls. Reminders fire offline, dose logs save offline. There is no drug encyclopedia and no interaction checker, so "offline" in those dimensions is "not present." For users who want a pure local reminder app with no cloud, MedTimer is the cleanest implementation.

3. Medisafe: reminders yes, drug info no

Medisafe's reminders are reliable offline, but opening the drug details or attempting the (premium) interaction checker without a connection returns errors. The pillbox metaphor and refill tracking still work, which covers the basic job for most users.

Same pattern as Medisafe. Reminders persist, vital-sign logs persist, everything clinical requires connectivity.

Dosecast's free tier works locally for reminders and logs. The cloud sync that restores data to a new device is a paid feature.

6. Mango Health: reminders yes, interactions need network

The LexiComp interaction data in Mango Health is fetched on demand. No connection means no interaction check. Reminders still fire.

Why the offline encyclopedia in MedRemind exists

The decision to bundle FDA and CIMA drug labels into the app was a trade-off. The install size is larger than apps without it. We accepted the trade because the users we talked to in rural areas, on international travel, and in hospitals with weak cellular signal explicitly asked for drug info that does not depend on a connection. The same logic drove the offline interaction checker.

What "offline reminders" really means on Android and iOS

All the apps above schedule local notifications through the operating system, which means the phone fires them with no server involvement. The difference is what happens when you tap the reminder, open the drug details, or add a new medication. MedRemind keeps those paths local too; the others take you to a spinner when the network is gone.

Where MedRemind beats the category
